1240-04-01-.10 Duty to Report Child Abuse and Neglect

The agency failed to submit a report to the Department of Children's Services when a parent brought concerns to them about their child. — Visit: Complaint Investigation,Technical Assistance — Action taken: CCR&R is scheduled to come out to the agency on May 29, 2026, to conduct training for all staff on incident reporting, duty to report child abuse and neglect, and active supervision. Room arrangement will be evaluated to ensure there aren't blind spots in the classrooms. The agency will also review policies for duty to report child abuse and neglect with all staff.

1240-04-01-.09 Incident Reporting

The agency did not submit a serious incident report for an incident that was required to be reported to the Department of Children's Services (DCS). — Visit: Complaint Investigation,Technical Assistance — Action taken: CCR&R is scheduled to come out on May 29, 2026, to conduct training for incident reporting, duty to report child abuse and neglect, and active supervision. Administration will complete a serious incident report anytime a parent brings concerns about an alleged incident, regardless of whether they are sure it happened or not.

1240-04-01-.11 Supervision

An educator was observed in the classroom door talking to someone in the hallway instead of focusing on the children. The educator was also observed on their phone. — Visit: Complaint Investigation,Technical Assistance — Action taken: The director spoke to each employee individually concerning cell phone use and standing in the doorway to talk to someone in the hall. This takes their attention away from the children and inhibits them from providing active supervision. Management has scheduled a staff meeting on December 8, 2025, and will review the requirements for active supervision, including the new licensing rules pertaining to the use of cell phones and electronic devices when children are present. The Small Business Coach from CCR&R will attend the staff meeting to assist.

1240-04-01-.11 Supervision

A child was moved from one class to another and was not signed in to the new classroom. — Visit: Complaint Investigation,Technical Assistance — Action taken: After the incident, the director spoke to each employee about ensuring children are signed in and out properly with all required information. The director wants to ensure all children are accounted for throughout the day and in the event of an emergency. The agency has scheduled a staff meeting for December 8, 2025, to formally review the requirements for children being signed in and out. Failure to abide by the requirements will result in disciplinary action for the employee. The CCR&R Small Business Coach will be inattendance at the staff meeting to assist.

1240-04-01-.22 Specific Requirements for Child Care Centers

During an unannounced QRIS monitoring visit, staff stated children were shuffled from one room to different classroom around 10:00am this morning. The agency did maintain adult:child ratios. Children shall be kept with the same group throughout the day. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency will schedule an additional floater to come in cover for when educators call out or are running late instead of shuffling children to different classrooms. The Small Business Coach from CCR&R is scheduled to conduct training/coaching on October 14, 2025, for staff schedules and what to do when staff call out.

An educator in the infant room did not document SIDS checks every 15 minutes as required. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The director and assistant director have started checking the infant classrooms throughout the day to ensure SIDS checks are being documented. The agency is working with CCR&R to have safe sleep training for staff.

1240-04-01-.09 Incident Reporting

A parent was not notified of an injury their child received at the agency. An incident report was not completed the same day of the incident. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The educator received information on incident reporting. CCR&R will conduct training with all staff on incident reporting. The tentative date is August 22, 2025.

1240-04-01-.15 Program Language and Literacy Development

An educator popped a child on the leg. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The employee was immediately terminated. The agency has a Professional Development Day scheduled for January 13, 2025, to receive training on challenging behaviors, developmentally appropriate practices, handling children with care, and infant/toddler positive discipline. All staff will attend. Child Care Resource and Referral will cover the training topics.

The educator did not wash her hands or the children's hands when they were finished. The educator was on her cell phone and not providing adequate mealtime supervision. — Visit: Follow-up visit,Technical Assistance — Action taken: Management met with the employee the same day as the incident to discuss the licensing rules for safe sleep practices, supervision, and food service. Since the incident, management has been checking in on the employee throughout the day to ensure she complies with the Licensing Rules. The agency held a staff meeting on November 12, 2024, to review the licensing rules for safe sleep practices, supervision, and food service/infant mealtime supervision. The Licensing Consultant also sent in a referral to Child Care Resource and Referral (CCR&R) to conduct one-on-one training and coaching with the employee.

In addition to training, all staff hired since May 1, 2024, will receive ongoing supervision coaching.
