Six (6) staff members did not complete all required training by the end of the QRIS year (9/30/2025). — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: We will ensure that staff start all trainings in a timely manner and follow through until completion of the required trainings are done. We will track all trainings through Pro-Solutions and maintain record keeping of all staff completed and uncompleted required trainings.

Upon interviewing the complainant and agency staff, the Licensing Consultant determined that on 4/14/2025, the agency did not inform the family that their child would be in the same classroom with the child that had bitten him on 4/10/2025. The agency had previously assured the family they would not be in the same classroom and that the parents would be contacted if it happened. — Visit: Complaint Investigation,Technical Assistance — Action taken: In the future, the director I will not make any promises that cannot be followed through. Knowing that any amount of time is not following a parent's request. Anything promised would need to be followed though 100% of the time going forward. The director reinterred that it to the admin team and teachers, so they are all on the same page going forward.
