An educator failed to conduct Face to Name roll call on January 19, 2026 and a child was left on the playground for approximately 3 minutes. — Visit: Complaint Investigation,Incident or Injury Investigation,Safety Plan Served — Action taken: The agency director has provided training for each educator on Name to Face procedures. She will be monitoring classrooms to ensure the procedures are being correctly implemented. Additional coaching will be provided by Child Care Resource and Referral Health and Safety Coach.

A supervision lapse occurred when a child was left unsupervised on the playground for approximately 3 minutes. — Visit: Complaint Investigation,Incident or Injury Investigation,Safety Plan Served — Action taken: The director has retrained all the staff in the appropriate way of face to name, that every single transaction will be documented. Meaning leaving the classroom, arriving at destination, leaving destination, and arriving back into the classroom. Every single time. Physically calling child's name and looking directly at them, nothing else matters until every single child is accounted for. The director will be checking face to name papers periodically throughout the day and noting on the transition sheets.

Staff member was using inappropriate discipline. — Visit: Safety Plan Served,Technical Assistance,Follow-up visit,Incident or Injury Investigation — Action taken: Facility will conduct a training and a follow up training with CCR&R.
