1240-04-01-.11 Supervision

During a safety plan visit LC observed 2 classrooms combined at naptime. The ages of the children were 29m-4years. Ratios for children six (6) weeks through thirty (30) months shall be maintained at naptime. — Visit: Safety Plan Monitoring — Action taken: Since the violation occurred, the nap time ratio mistake was corrected and combining the two age groups has not taken place since that time. The licensing rule was fully explained to the Interim Assistant Director and also reviewed and shared with staff. All ratio charts were checked to ensure that the November 2025 version was the version posted in all classrooms (they were correct). This ratio will be intentionally focused on during onboarding new staff during orientation to ensure that we all acknowledge and understand this rule.

1240-04-01-.09 Incident Reporting

During a follow up visit a parent came in upset that there was not a report about the bruise and scratches on the child's right arm. — Visit: Safety Plan Monitoring,Follow-up visit,Technical Assistance — Action taken: Incident report was filled out and given to parent. CCR&R will be conducting training in the are of incident reporting and health and safety the importance of well child checks upon entry of the classroom.

1240-04-01-.11 Supervision

During a follow up it was witnessed by FS Smith that breakfast had been served to the children and the educator was in the storage closet gathering materials for breakfast. FS Smith gave strong TA about mealtime supervision. — Visit: Safety Plan Monitoring,Follow-up visit,Technical Assistance — Action taken: CCR&R has been scheduled to conduct trainings in supervision specifically in meal time supervision. Educator was given TA and the issue was resolved at time of visit.

1240-04-01-.08 Record Keeping

During a follow up visit children's file were not ready availble for TDHS to review. Access has not been granted to the interim director. — Visit: Safety Plan Monitoring,Follow-up visit,Technical Assistance — Action taken: TA was given to interim director and a plan is in place to make sure the interim director and TDHS has access to all staff and children's files. CCR&R will slao be conducting trainings with admin. focusing on managment responsiblites.

71-3-508. Inspection of Entities Providing Child Care

During a complaint investigation while LC was conducting investigation, there were several incidents where subs from Uriah were being left alone with children. The safety plan states that there shall be a Martha O'Bryan educator always present. — Visit: Complaint Investigation,Technical Assistance — Action taken: TA was given to the interim director about the importance of following the current safety plan.

1240-04-01-.09 Incident Reporting

During a complaint investigation it was discovered that parents have made threaths to employees on several occasssions and this was not reported to TDHS as an incident. — Visit: Complaint Investigation,Technical Assistance — Action taken: CCR&R will be conducting training on incident reporting.

1240-04-01-.22 Specific Requirements for Child Care Centers

During a complaint investigation it was discovered that educators have been out of ratio for periods of times, mainly in the mornings. — Visit: Complaint Investigation,Technical Assistance — Action taken: There wil be hourly checks for ratio. Licensing will conduct a training on ratio and grouping with entire staff and adminstration.

1240-04-01-.18 Emergency Preparedness

During a complaint investigation it was discovered that protocol in the Emergency Prepardness Plan has not been followed when threats have been made on educators and the center. — Visit: Complaint Investigation,Technical Assistance — Action taken: CCR&R will conduct a training going over the importance of the Emergency Prepardness plan. There will be a review of the current EPP.

1240-04-01-.11 Supervision

During a complaint investigation it was observed that children came in from the playground and name to face was not conducted on the playground nor in the classroom. — Visit: Complaint Investigation,Technical Assistance — Action taken: TA was given immediately. CCR&R will be conducted training with all staff on supervision during transition times and proper name to face.

1240-04-01-.06 Staff

During a complaint investigation it was discovered that an educator was in a classroom without the mandatory training. Educator was pulled from classroom. — Visit: Complaint Investigation,Technical Assistance — Action taken: The educator was removed from the classroom immediately and was given the opportunity to complete the mandatory training. TA was given to the interim director and support staff. CCR&R will also be conducting trainings for managment responsiblities.

1240-04-01-.12 Health and Safety

During a complaint investigation. LC was informed that a 2.5 year old had ingested essential oil. This bottle is labeled keep out of reach of children. — Visit: Complaint Investigation,Technical Assistance — Action taken: TAM referral for health and safety for CCR&R to come and do on going coaching in the classroom. All diffusers and essential oils have been removed from the classroom.

1240-04-01-.22 Specific Requirements for Child Care Centers

During an unannounced investigation the LC observed children being shuffled in classrooms to maintain ratio outside of the 1 hr. window of the opening of operation. — Visit: Complaint Investigation,Technical Assistance — Action taken: TAM referral for CCR& R to do trainings. Each classroom now has assigned educators. New hires will be trianed before being left alone in a classroom without a lead Martha O'Bryan educator.

1240-04-01-.11 Supervision

During a complaint investigation LC was notified that two children 2.5 years of age had gotten into the essential oils. One had ingested the oil and the other contaminated his hands, face and eyes. One educator was performing diaper changes and the other was with other children. The children had entered an area where children should not have access. — Visit: Complaint Investigation,Technical Assistance — Action taken: TAM referral was put in and CCR&R will be conducting trainings and ongoing coaching. Administrative will be conducted weekly checks for " keep out of reach children" items in the classroom. All of the diffusers and essential ils have been removed from all the classrooms

During QRIS monitoring LC witness educator place snack garnola, yogurt and water on the table and then take children outside . Snack was placed on the table at 2:24pm. Children returned back to have snack at — Visit: QRIS Monitoring visit,Complaint Investigation,Technical Assistance — Action taken: The educator was retrained by our kitchen staff and management on the proper techniques and times that food can be set out for. Also, CCR&R will train on food and food services and continued support.

During complaint investigation and QRIS it was discovered that incidents have not been reported to parents no later than the close of business day. — Visit: QRIS Monitoring visit,Complaint Investigation,Technical Assistance — Action taken: Staff has been re-trained on how to properly record any incident, including but not limited to the child that was hurt but also the child that caused the incident. Also, CCR&R will be conducting on going training, coaching, and support in the area of incident reporting

During complaint investigation it was determined that there is a discipline issues causing children to be injured. Educator is not using proper techniques to intervene in the situation. — Visit: QRIS Monitoring visit,Complaint Investigation,Technical Assistance — Action taken: Staff member has been spoken to about intervening earlier to prevent or to stop the actions that are causing children to get hurt. Management will be doing daily observations to ensure this taking place. CCR&R will conduct training on classroom managment and behvioral managment.

During serious incident investigation it was determined that an educator phycially disciplined a child and it was witnessed by two educators. — Visit: Incident or Injury Investigation — Action taken: Staff memeber was terminated and other staff was reviewed on policy form MOB and TDHS on proper discipline procedures
