Name to face was conducted in the gym and educator Mandy Vincente led children to the playground. A child was left in the gym as the class transitioned to the playground for approximately 3-4 mins. Name to face was conducted again at the playground and educator realized the child was not there. Educator went back with her class towards the gym and intercepted another adult who was bringing the child to her from the gym. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: LC received the incident report. LC went to the center today and investigated the incident. LC issued a violation and sent a safety plan for legal to approve.
