1240-04-01-.11 Supervision

On May 1, 2025, LC Hobbs visited the agency to investigate an incident that occurred on April 28, 2025. During the monthly fire drill held after nap time, one child remained asleep and was inadvertently left behind during the building evacuation. The "Name to Face" check was not completed when leaving the classroom. The educator noticed the child's absence during the roll call conducted outside, and the director promptly re-entered the building to retrieve the child, who had been alone for approximately two minutes and was still asleep when found. — Visit: QRIS Monitoring visit,Incident or Injury Investigation,Technical Assistance — Action taken: CCR&R will conduct a supervision training for all staff, and the director and assistant director will observe all classrooms during transitions to ensure name-to-face roll call is being completed.
