Evidence gathered by verbal admissions from multiple agency staff during LC Eldridge's investigation has confirmed that a serious incident occurred on June 9th, 2026 involving a 1-year-old child walking off of the playground without the knowledge of the staff and was left unattended for less than 2-5 minutes. The child was retrieved by an educator that was counted in ratio in the infant room. When the educator went to retrieve the child the infant it caused the infant room to be out of ratio with only 1 adult and 5 infants. — Visit: Incident or Injury Investigation — Action taken: The caregiver that retrieved the child was educated on the importance of maintaining ratios at all times. Locks and signs were placed on each gate on the playground. Staff have been educated on the importance of playground checks before utilizing the playground, the importance of supervision, and maintaining ratios at all times.
