Licensing Consultant (LC) conducted an unannounced visit on Tuesday, October 28, 2025. LC viewed the child's file and did not see an incident report regarding the incident with the child having a carpet burn. LC inquired about the incident and the director reported that the incident happened and she thought that an incident report had been done. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The provider has put procedures in place to ensure that the educators write incidents up and make two copies and file one for the center record and send one home to parent. The provider will also ensure that someone is in charge or in the office to make sure that it in put in the file in office.
