    During an unannounced follow-up visit, while conducting ratio checks LC observed one infant sleeping in a crib on their stomach with a bib by their head. — Visit: Follow-up visit,Technical Assistance — Action taken: The Director stated she will conduct random walk-throughs of the infant classroom to ensure all Safe Sleep Practices are followed. The entire staff is also attending Safe Sleep training with CCR&R to ensure all staff are well-versed in this area of care.

Educator One was still assisting the children washing hands and going to the restroom. There was not an educator supervising the remaining children eating at the table. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): Providing continuous mealtime supervision training to all staff. The director will also conduct random mealtime supervision checks for the next two weeks for all classrooms.
