    0520-12-01-.06 Program Operations. (6) Each program shall have a written playground supervision plan that includes: (b) Supervision assignments of staff to assure that all areas of the playground can be seen so that all children can remain within sight of the teachers; (c) Identification of which staff will merely supervise in their assigned zone while other staff, if any, interact with children as play facilitators. An incident occurred between two children on Thursday, April 23, 2026. One child was going down the slide while the other child was going up the slide. The child going up the slide wanted the child going down to move, that child refused. It resulted in the child going up the slide to bite the child going down on the private parts. While investigating the incident I learned that staff were not stationed to observe all areas of the playground and was not there when the incident occurred. On April 30, 2026, the corrective action plan was returned by the Director with the following measures implemented to prevent the incident from recurring. Staff have completed training in outdoor safety, health and safety policies and procedures, and the "Thumbs Up" supervision strategy. Certificates verifying completion of these trainings were provided. The program also submitted a playground policy, a playground supervision plan, and a playground diagram indicating where each teacher is assigned while supervising children outdoors. — Visit: Non-CCDF,Incident or Injury Investigation — Action taken: The corrective action plan was returned by the Director with the following measures implemented to prevent the incident from recurring.
