During an unannounced follow-up visit, LC determined that it appeared that floors are not kept clean and observed tables in one of the classrooms that still had residue from other materials used on the table. — Visit: Follow-up visit — Action taken: To prevent the provider from receiving this violation again, the tables and chairs will be cleaned before and after use. The provider will also find products to remove residue from tables and chairs.

During an unannounced follow-up visit, LC Kelley and the director walked all classrooms and there were several outlets that did not have protectors. The director stated that some of those already had the safety on and would reject the outlet protection covers. — Visit: Follow-up visit — Action taken: To prevent the provider from receiving the violation in the future, a walk-through will be conducted to ensure there are outlet protectors.

During a unannounced visit to investigate a complaint LC Utley completed a walk through of each classroom it was determined that a few of the classrooms had outlet covers that were missing. This had been discussed with director by the LC on previous visits to the agency. — Visit: Complaint Investigation — Action taken: All classroom has been supplied with a box of cover to replace any that may be missing. The teachers will make sure to replace any outlet covers that may have been removed for usage

During an unannounced visit on 5/15/26 to investigate a ratio complaint after LC Utley reviewed the name to face roll call sheets it was determined that on 5/14/26 the infant class was out of ratio for a period of 16 minutes. The ratio for that class is 1:4. The class had a ratio of 1:5 during that time. — Visit: Complaint Investigation — Action taken: We have changed the schedules of some of the educators to assure that we have full coverage for all classes during our drop off period. All educators now have walkie talkies to inform management when they are in need of assistance regarding ratios.

During an unannounced visit on 5/15/26 to investigate a ratio complaint after LC Utley reviewed the name to face roll call sheets it was determined that on 5/13/26 the infant class was out of ratio for a period of 56 minutes. The ratio for that class is 1:4. The class had a ratio of 1:7 during that time. — Visit: Complaint Investigation — Action taken: We have changed the schedules of some of the educators to assure that we have full coverage for all classes during our drop off period. All teachers have been given walkie talkies to call for help when class reaches ratio.

During a unannounced visit to investigate a complaint after LC Utley completed a walk through of each classroom it was determined that in some of the classrooms tables are not cleaned after meals, floors not being swept after meals, sinks that have dried soap and dirt in the them and children's restrooms that are not clean. — Visit: Complaint Investigation — Action taken: Cleaning walkthrough with all staff members to assure that they all have a clear understanding. To prevent this from happening again all staff will start using a classroom checklist.

During an unannounced visit on 4/20/26 to investigate a ratio complaint in the infant class. While gathering ratio's LC Utley observed an child eating snack the supervising educator Nicole R. seated in a chair with her back to the child that was eating. While the child was still eating the educator was giving instructions to the other children in the classroom. — Visit: Complaint Investigation — Action taken: The agency will have a Meal Time Supervision training for the entire staff and also we have a CCRR Rep coming to do a training Director and Asst Director will monitor all meal times to prevent this from happening again.

There was an educator working in a classroom whose background check had expired. The educator did not get reprinted within the 5 year period as required by licensing rules. — Visit: Complaint Investigation — Action taken: he educator has gone to take her fingerprint and it has returned and cleared. Going forward we will do a continuous check on all educators background before they expire

During an unannounced visit on 4/20/26 to investigate a ratio complaint in the Infant class. While completing ratio checks LC Utley observed educator Nicole R. who was the only educator in Pre K class sweeping the floor while a child was eating snack. — Visit: Complaint Investigation — Action taken: All meals will be supervised by Director/Asst Director to avoid having another violation. The agency will have a Meal Time Supervision training for the entire staff and also we have a CCRR Rep coming to do a training

During an unannounced visit on 4/9/26 to investigate Food service and Inappropriate Discipline complaints after LC Utley spoke with the director Tamara M. who had spoken to the Kemarion S. the educator named in the complaint the educator stated he did grab the child's arm but not forcefully. The director also asked the child about what happened. It was determined that inappropriate discipline was used on a 4 year old child. — Visit: Complaint Investigation — Action taken: The action taken is that the teacher was terminated for inappropriate discipline of a child. We will have a staff meeting on 4/22/26 to go over licensing rules & regulations...also to re-train all teachers on appropriate teacher child interactions.

During a visit on 3/5/26 to investigate a Physical Abuse complaint, while on the playground LC Utley observed Name to Face Roll Call not being done when children arrived on the playground and when another class was leaving the playground. — Visit: Complaint Investigation — Action taken: We are retraining all staff members on Face to Name transitions, we have set up trainings through our company. To prevent this from happening again management will do daily checks during playground transitions for all teachers to assure that we are calling names and not counting. We have posted the One Foot In and One Foot Out poster.

During a complaint investigation LC observed an educator in the Twos classroom, changing a diaper and not washing the child's hands or her own hands after changing the child. Specifically diapering procedure.

In Toddler B class there was lysol, air freshner, and other cleaning products that stated " KEEP OUT OF REACH OF CHILDREN" sitting on counter. LC removed them. — Visit: QRIS Monitoring visit — Action taken: Site director has went through all rooms removing any items that are meant for "KEEP OUT OF REACH OF CHILDREN" these items have been locked away.

During an unannounced QRIS visit classroom preschool A 2.5-3 was using 1/2 staff for nap time, but children were not ready for resttime. Several students were awake and moving around the room. Second edcator is needed until children are resting. — Visit: QRIS Monitoring visit — Action taken: LC has requested CCR&R for review and ongoing training in supervision areas this includes naptime supervision. Provider has reviewed the rules with educators. Provider will ensure 1/2 staff for 31mths and up during naptime will only be used when children are completed relaxed.

During an unannounced QRIS visit several cots did not have clean sheets covering them. Cots were not labled for use. — Visit: QRIS Monitoring visit — Action taken: Provider has reviewed the license rule of bedding and has ensured that all cots will be covered properly and labled.

Menu was posted however it did not reflect what was served to the students. -Menu for today for lunch was: Turkey sandwich, turkey slices, whole grain bread, mixed vegetables, pineapple, 1% low fat milk. Meal served was peanut butter and jelly with carrots and peas, blue berries. There was not write in on the menu that is posted for food items changes — Visit: QRIS Monitoring visit — Action taken: Provider will ensure menus are posted for review of parents and that what is served will reflect them items on the menu. If there are changes to menu this will be noted on the menu for parents review
