Accurate attendence records were not maintained in the classroom where children where being cared for. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency will update sign in sheets if classrooms have to combine due to low census and will inform parents via Brightwheel of the switch.

The primary educator has been employed for 9 months and has not completed the TNELDS training. This training is required to be completed within 4 months of employment. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The provider will ensure the employee completes the required training before returning to work. The provider will furthermore ensure all employees complete the training before starting work.

DHS staff confirmed that children under the age of 31 months were routinely combined with older children as part of the daily afternoon schedule which is not in compliance with the rules. — Visit: Complaint Investigation,Technical Assistance — Action taken: Schedule will now explicitly state that children are only combined the last 30 minutes of the day if they are 2.5-5 years.
