On 01/15/2026, the educators were not in compliance with the mealtime supervision plan. The license consultant observed in the toddler classroom that neither educator was sitting at the table while the children were still eating. During outside play two (2) educators from the same classroom left their children unattended. A child from the same class was also observed running around in the classroom with no supervision. — Visit: Technical Assistance,Complaint Investigation — Action taken: The agency corrected the violation with the following action(s): Educators involved in incident received disciplanary actions. Additional training on supervision will be conducted. When an educator need to leave the class, management will be notified, ensuring supervision is always in compliance. The agency will be placing partition doors in the open classrooms to keep the children from wondering. The agency is in the process of hiring additional staff to help to make sure supervision is covered at all times.

The child was safely placed back with his class. The agency will be adding partitions in the open classrooms to help keep the children from leaving. A plan of communication is in place if the staff needs a break. The staff will have training on supervision.

Once the two educators left this made the teacher/child ratio 2:30 which is non-compliant.
