A staff member gave a child an extra dose of medication. — Visit: Incident or Injury Investigation — Action taken: After being notified of the incident by the center, the Health Program Manager and the Head Start Nurse met with the Program Director and Associate Director to review the situation and determine appropriate corrective action. The team examined the existing medication administration protocol to identify any gaps that may have contributed to the incident. Disciplinary memos were issued to the two staff members involved to underscore the seriousness of the situation. To prevent future occurrences, several measures will be implemented. The family specialist completed the Administering Medication Training through ProSolutions on May 15, 2025. Medication bags will be relabeled to clearly indicate the total number associated with each child (e.g., "Bag 1 of 2") and will include the medication name on the outside label. Documentation of the training provided by the Head Start Nurse will be enhanced to ensure clarity and accountability. Future medication administration trainings will include a detailed review of the "Five Rights of Giving Medication" using a distributed flyer. Additionally, the training will be updated to place stronger emphasis on the responsibilities of the Witness role to ensure a clear understanding of their duties during medication administration.
