An infant was observed waking from a nap in a crib with a bib on. Nap log checked, and a 15-minute nap check was missed which is a violation of Safe Sleep Procedures. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Safe Sleep corrected on site, bib was removed, and the child was taken out of the crib. The Licensing Consultant reread and reviewed safe sleep practice/regulations for infants with the staff in the room. CCR&R training on safe sleep scheduled for December 15, 2025. Procedures for Safe Sleep will be maintained by using a 10-minute timer and documenting nap checks in BrightWheel. CCR&R will continue ongoing coaching in the infant classroom with the director performing unannounced walkthroughs daily, and coaching staff on Safe Sleep Procedures.
