    On 12/29/25, the agency self reported that six toddlers, aged 13- 17 months, were left unattended for approximately 35 minutes. During the complaint investigation on 12/30/25, LC Hawthorne found that during nap time, the room was dark and the white noise machine was playing loudly. An educator alerted her co-educator that she was going on break. The co-educator did not hear or see that the educator left the room due to how dark the room was and the loud white noise machine. The second co-educator also left the room to go on break. The educator returned from break, finding that the children had been left unattended. — Visit: Complaint Investigation — Action taken: Retraining re: supervision, safe sleep, and appropriate noise and light levels will be completed by all staff. The agency has established a hard copy sign-in / sign-out log for all breaks.
