Incidents need to be reported the day of the incident to the parent and to DHS. 1240-04-01-.10 Duty to Report Child Abuse and Neglect

Center has a duty to report all suspected Child Abuse to DCS. — Visit: Complaint Investigation,Incident or Injury Investigation,Technical Assistance — Action taken: The center said they were not aware of the correct process and has been briefed on such. They will now reporting all suspected abuse to DCS first.

Agency did not have the proper medciation forms or documentation to administer prescribed medication to a child. — Visit: Technical Assistance,Complaint Investigation — Action taken: Center stated they will have Policy Review & Retraining: All staff members will be reviewing the agency's medication administration policy in full. They have also implemented a revised medication log.

Not all staff did not completed the required training. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Verify monthly the training of all the staff to make sure that be done by the due date.

The agency did not keep the parent notified/informed of her child's daily activities (mood, time out, etc. — Visit: Technical Assistance,Complaint Investigation — Action taken: We had a touch base with all our teacher and explain the process to them again. They are practicing how communicate with parents in person and through ProCare every day.

An educator was inappropriate when disciplining a 3-year-old child. — Visit: Incident or Injury Investigation,Complaint Investigation,Technical Assistance — Action taken: We do not agree with or allow any form of physical discipline or inappropriate contact with children. We are committed to reviewing the footage and addressing the matter in accordance with our policies and procedures, ensuring the safety of all children and fairness to our staff.

A serious incident occurred at the agency and there was no incident report to the parent, director not notified, and DHS not notified. The class was walking down the hallway to the gym as a group and one child had a temper tantrum. While the educator was attending to this child the other children left the group. At that time those children were unsupervised. — Visit: Complaint Investigation,QRIS Monitoring visit,Technical Assistance — Action taken: The educator was instructed to call for help on her walkie when losing control. And that she also could have called the walking children back to her instead of allowing them to walk in front of her.

A 4-year-old male child was left alone on the playground for 2 minutes. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: All staff were trained on name to face and all new staff will be on-boarded with name to face training during orientation and before starting in the classroom.

1240-04-01-.16 Physical Facilities

The outside area that has active construction is very close to the fenced in playground area and can be accessed by children if unsupervised. There was no barrier to separate the spaces. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: A temporary fence has been bolted in the main ground to only allow passage from door to playground. A lock was placed on the toddler playground that they can only enter and exit through the main playground door.

Two of four children's files stated the children have peanut allergies, but two of these children's files did not have the required written plan of action endorsed by the child's pediatrician or licensed medical provider. — Visit: Complaint Investigation,QRIS Monitoring visit,Technical Assistance — Action taken: We informed parents that we are a nut free center. And updated doctor instruction were place in children files.

1240-04-01-.12 Health and Safety

A child in the 3-year-old room needed medication administered and there were new educators, that had not been trained on this child's medication, providing care for the child that date. — Visit: Complaint Investigation,Technical Assistance — Action taken: All staff will take medication training before beginning/ giving mediation to a child. This will be checked annually to keep updated.

An educator administered medication without having already been trained in medication administration. — Visit: Complaint Investigation,Technical Assistance — Action taken: We updated our Orientation training checklist to include taking the medication training. We will check annually for classroom staff to take the medication training before beginning/ administering to a child.
