    An employee walked off the job and left three four-year-olds and three five-year-olds unsupervised during nap. The supervision lapse lasted for less than a minute. The coteacher was returning from lunch and passed the departing employee in the parking lot. The children remained asleep for the duration of the lapse. — Visit: Complaint Investigation — Action taken: The situation was conferenced with DHS staff and it was concluded that the supervision lapse was isolated to the one individual who walked off the job. That individual will not be rehired; so it will not be necessary to retrain all staff. The CAP is complete and the TAM referral has been cancelled.

    On Monday, November 3, at approximately 1:00 PM, an educator walked off the job and left 3 four-year-olds and 3 five-year-olds sleeping. The co-teacher was coming back from lunch and passed her in the parking lot. The children were left alone for less than one minute. — Visit: Complaint Investigation — Action taken: A safety plan has been put in place so that the educator may not be rehired at 21st Century Child Development Center or any other DHS licensed agency until she retrains in supervision procedures. All staff will retrain in supervision procedures.
