On 6/9/26, an educator walked out of the classroom while 2 children were still at the table eating. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Employee was transferred to another center and put on a 90 probation on Friday the 12th of June. Employee was terminated on Monday June 15th after seeing additional child left unattended in the video from 6/9 and not showing up for work at new location. Agency will go over supervision with current staff and remind them to be aware of meal time supervision plans.

On 6/9/26, an educator walked out of the PreK 2 classroom, leaving 11 children ages 3 year 6 months to 5 years old unsupervised. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Owner self reported as soon as she found out. The employee was put on probation for 90 days, put on a safety plan and move to a different center. The employee was terminated due to attendance. We remind staff the importance of never leaving the room unsupervised and will provide additional training to any staff who feels they don't understand the rules or thinks they need a refresher.

An educator left a child in the bathroom unattended for 3 minutes. — Visit: Incident or Injury Investigation,Safety Plan Served,Technical Assistance — Action taken: The employee was written up and CCR&R will come in and do extra training with the employee.

An educator left a four year old child in the bathroom unsupervised for 7 minutes. — Visit: Incident or Injury Investigation,Complaint Investigation,Technical Assistance — Action taken: Employee was put on a safety plan to recieve training and there will be extra training for all employees in the center.
