    An educator left the playground leaving 1 educator present with 15 two and a half to three year olds; therefore ratio was not maintained. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Management has gone around to all current employees to let them know that they are not to leave a classroom if they are counted in ratio. They are to call the front desk to reach a member of management or another available staff member to step into the classroom if they need to leave for any reason. Management will supervise transitions in and out of the classrooms to help with Name to Face and making sure all staff members are present in the room when counted in ratio.

Management and employees will resign our Name to Face policy and review our sign in procedures when a child is dropped off and placed in our care.
