During a complaint investigation it was discovered that information in the complaint stating children being "hit with broom" " Hit up against the wall" is valid — Visit: Complaint Investigation — Action taken: CCR&R conducted training and ongoing coaching in the area of impropper discipline and child abuse and neglect. Provider has implmented coreective action for employess that do not follow the license rules and regulations

During an unannounced visit, LC Kelley conducted interviews and it was determined that an educator handled children in an inappropriate manner. — Visit: Complaint Investigation — Action taken: CCR&R conducted training and ongoing coaching in the area of proper discipline techniques. Provider has also implmented zero tolernce policy for staff and improper dicipline actions.

During a complaint investigation it was discovered that an incident occurred with a lapse of supervision and an incident report was not written nor were parents informed by the end of the day. Provider will be live viewing classroom to make sure all educators are supervising children

Safe sleep procedures have been added to each classroom for 2yrs and younger.

LC Kelley observed a medication for a student that expired in January 2025. — Visit: QRIS Monitoring visit — Action taken: The provider has established a form that will be updated with expiration dates. This form is checked on a monthly basis to ensure none are expired.

During an announced visit, it was discovered that staff did not complete training hours for the QRIS year 2023-2024. — Visit: QRIS Monitoring visit — Action taken: The director will ensure that staff completes at least 2 trainings each month.
