After seeing the camera footage, it was determined that an educator observed suspected child abuse and did not report the concerns to the correct agencies as required. — Visit: Complaint Investigation,Technical Assistance — Action taken: Admin and all staff will recieve training on duty to report child abuse and neglect & how to report it.

During the PreK classroom observation, the class was outside on the playground. Name to Face roll call was not done when lining up to go inside. Name to Face Roll call wasn't done upon returning to the classroom. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Director will meet with the staff about the importance of name to face and staff will recieve training refresher on name to face roll call.

The max group size for 2 1/2 -5 years old is 20, there were 24 children present in the Drums classroom — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The children were immediately moved to meet the required group size. The educators will be trained to know their group ages, ratio, and maximum room capacity.

A substitute was present without a record of their names, addresses, telephone numbers, and hours and dates of service — Visit: Follow-up visit,Technical Assistance — Action taken: The agency sent the substitute home until they had everything accessible before utilizing the service again.

1240-04-01-.15 Program Language and Literacy Development

In the Trombone classroom a child was sitting in a table chair by themselves. The rest of the children were sitting on the floor listening to the educator reading a story. The other educator was holding a child. They had placed the child in the table chair to learn how to sit so they would not run off, the child had been secluded at the table for at least 5 minutes. — Visit: Follow-up visit,Technical Assistance — Action taken: The child was removed from the chair and the importance of sitting with the group was explained to the educators. Further training from CCR&R will be provided to the educators.

The Tuba classroom was running a 1:9 ratio with only 2-year-olds present. A second educator was added to meet ratio, but the educator left the classroom. — Visit: Follow-up visit,Technical Assistance — Action taken: The agency will review and continue to ensure educators are aware of the ratio, and they must be followed.
