During the announced visit on 11/18/2024, the Licensing Consultant (LC) observed the swing and slide set was not securely anchored while the children were playing. LC also observed loose screws and bolts on the swing and slide set. The wooden floor board at the top of the slide had loose wooden planks. The wooden border that surrounded the swing and slide set was not secured. LC observed a child walking on the wooden boarder and it moved allowing the child to almost slip and fall. The wood chips and rubber mulch measured between 3-4 inches around the slide and climber wall. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The wooden border, swing and slide set was removed from the playground.
