Management will oversee proper transitioning from all staff going to and from the playgrounds until everyone is completing name-to-face properly and following the playground supervision plan correctly to ensure that all children are supervised properly. A formal incident report was neither completed nor provided to the parent regarding the child who was left unsupervised on the playground for approximately 30 minutes. — Visit: Complaint Investigation — Action taken: A formal incident report will be completed & provided to the parent regarding any child injury, incident, or accident. A report will be submitted to DHS/ DCS as well. Management will document any and all incidents, injuries, accidents and illness to parents and obtain parent signatures. Walkie talkies are now being used by staff to contact front desk before going outside and before coming back inside so that management can help assist and make sure name to face is done properly ensuring all kids are accounted for upon transitioning to and from playground.

Staff engaged in inappropriate discipline, including screaming in children's faces, failing to comfort distressed children, and making derogatory comments. For infants/toddlers: The educator shall hold and comfort children that are upset. (c) Potentially shaming, humiliating, frightening, verbally abusive, injurious discipline methods, and/or techniques that isolate the child are prohibited. — Visit: QRIS Monitoring visit — Action taken: Management will host a staff meeting on April 3, 2025 and will retrain ALL staff on importance of providing a loving and nurturing environment. Managment will also instruct all new educators upon hiring (during the orientation) the importance of being a nurturing and loving educator. Management will actively observe staff in classrooms and assess their demeanor and disposition. Management will ensure that all educators know to ask for help or ask questions to better handle behaviors schedules, etc. Mangement will follow up with staff when hired, after class changes, and throughout employment to assess their behavior and demeanor with children. If any negative behaviors or lack of nurturing are observed, management will address with staff immediately and educator will be written up & provided additional training and/ or suspension or termination. All staff will be retrained by CCRR as well.

1240-04-01-.15 Program Language and Literacy Development
Children were not dressed appropriately for the weather, exposing them to unsafe conditions. In addition, failure to change diapers in a timely manner resulted in cases of diaper rash and discomfort. — Visit: QRIS Monitoring visit — Action taken: All staff will be trained/ retrained on outside procedures and policies. All educators must call the front desk before going outside to confirm that the temperature & conditions are good for the class to go outside. At that time, management will confirm that all children are appropriately dressed for the conditions. (coats, shoes etc.) This will be implemented throughout the school. Management will make sure all staff and parents continue to bring proper clothing & shoes for children. When weather conditions are not within state guidelines for outside time, teachers will implement indoor gross motor activities. All staff will be retrained on diapering and hand washing procedures. Management will walk through classrooms consistently and observe that staff are following proper procedures. Management will also check our app (Procare) to make sure that diapers are being changed and logged at least every 2 hours. We are having a staff meeting/ training on April 3rd to review violations and retrain staff.

1240-04-01-.09 Incident Reporting
On September 12, 2024, It was reported that one educator had "thrown" the children in her class on multiple occasions. Allegedly, last week, a child was grabbed by the collar of his shirt and scratches were left on his neck. The report also stated that these incidents have been reported to the director multiple times with no action taken and no incident report was given to the parents. On September 13, 2024, LC Powell made an announced visit to interview all staff involved in this incident. Investigation findings revealed that there was no written incident report on file for the child or educator and the parents had not been notified. On September 16, 2024, LC served an individual safety plan for the educator in compliance with TN childcare licensing rules and regulations. On September 17, 2024, LC received notice that the educator was terminated from this center. This incident is pending further investigation by the SIU. — Visit: Safety Plan Served,Complaint Investigation — Action taken: The parents of all the children mentioned in the complaint were notified. The educator named in this incident was terminated. The center will contact CCR&R for further training on effective classroom discipline and management. The Director will also have all educators take the ProSolutions training, Keep Calm and Try Positive Guidance and Redirection. Management will walk the building more often each day and sit in each room to observe the educators making sure to correct any type of teaching or behavioural techniques as needed.
