During an unannounced visit to investigate a complaint, after LC interviewed director it was determined that a Serious injury report was not made to DHS. The child's family informed agency that the child was taken to the hospital for medical care following a fall in the classroom. The agency was informed of hospital visit the next morning when child returned to school. — Visit: Complaint Investigation — Action taken: Document: Clear procedures and incident forms. Train: Ongoing education for staff on reporting requirements. Escalate: Immediate notification and action by the director. Audit: Regular reviews of compliance and follow-up processes. Communicate: Transparent communication with parents about expectations.
