    During an unannounced visit to investigate a serious injury incident, LC observed a 5 year old child brought inside by an educator to use the restroom. The child went into the class then into the restroom without an educator present in the classroom. The educator sent the child into the classroom alone. The child was left unattended in the restroom and classroom for approximately 2 minutes until educator went into restroom area with child. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: To ensure that children are supervised at all times, the Director has reviewed and distriubuted the Supervision section of the rules to all educators. A verbal reminder of the rules will be given weekly to all staff by the director, and formal review of the rules will be covered during therir next staff meeting on May 22, 2025.
