1240-04-01-.07 Criminal Background Check and State Registry/Records Review Procedures

On 8/14/25,LC Chastain and FS Pass completed an unannounced visit. During a complaint investigation it was determined that Courtney Dye, whose criminal background check identified an unknown excludable criminal offense had been working at the child care agency while children were present. — Visit: Safety Plan Served — Action taken: The director will ensure that the excluded person is not performing any work at the agency during the hours that care is provided to children.

On 8/14/25, LC Chastain and FS Pass made an unannounced visit to serve a safety plan. The owner(Kimberly Porter) is responsible for insuring that all staff working with children or while children are present have passed a background check. Courtney Dye was excluded from working with children for unknown reasons. As part of a complaint investigation, two parents reported that Courtney Dye was working at the agency in an educator role. — Visit: Safety Plan Served — Action taken: N/A

On 8/12/25, LC Chastain and FS Pass completed an unannounced visit to investigate a complaint. It was found that the staff medication were located on top of a shelf in the owner's office. Staff medications were stored on top of a high shelf some were stored in a plastic container. The medications were located in the owner's(Kimberly Porter) office. During an audit by the Child Care Compliance Program conducted from 2/20/25-5/13/25, it was found that not all sign in and out sheets provided were completed accurately. There was a total of one thousand four hundred one (1,401) errors identified for thirteen (13) children from the test period of May 1, 2024, through February 8, 2025 — Visit: Safety Plan Monitoring — Action taken: The agency has completed training of staff and updated the procedures realted to sign in and out policies.
