Two children escaped the gross motor area and ran down the hallway unsupervised. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The educators for the classroom were immediately terminated. The other educator was counseled. The agency is having a staff meeting on 11/26/2025 to review supervision. The administration is developing a specific supervision plan for the indoor play area, and it will also be reviewed with staff. The Child Care Resource and Referral training partners will also provide training on Active Supervision and Name-to-Face Roll Call.
