1240-04-01-.11 Supervision

During an unannounced QRIS visit LC received a complaint that a 18 month old child was left unattended in the hallway of the agency for approximately 8-10 minutes. — Visit: QRIS Monitoring visit — Action taken: After the child was returned to his classroom, both teachers involved were interviewed to better understand what occurred and to explain the seriousness of the issue. The leadership team developed a safety and supervision plan which was reviewed and signed by all educators. Name-to-face procedures were reviewed, modeled, and clarified. Additionally, younger toddlers' transitions were reduced and the Lead Teacher has been designated as the responsible person for their trips out of the classroom. Classroom doors will remain closed unless a group is actively entering or exiting the classroom.

1240-04-01-.09 Incident Reporting

When investigating a complaint LC observed that an incident report was not completed the day the incident occured. The incident was not reported to parent the same day it occured. The incident was not reported to DHS or DCS the day it occured. — Visit: QRIS Monitoring visit — Action taken: The supervision incident was discussed with parents of child involved, and an incident report was completed and signed. The incident was discussed with the licensing consultant during her visit and an understanding of the proper protocol to follow was explained and understood.

Management was made aware of an Lapse of Supervision incident but did not report it to DHS or DCS. — Visit: QRIS Monitoring visit — Action taken: The supervision incident was discussed with parents of child involved, and an incident report was completed and signed. The incident was discussed with the licensing consultant during her visit and an understanding of the proper protocol to follow was explained and understood. Teachers were interviewed to determine how the lapse in supervision occurred. A comprehensive safety plan was developed. Name-to-face procedures were modeled and observed. The younger toddlers' transitions out of the classroom have been reduced and will be managed by the lead teacher.

1240-04-01-.06 Staff

After staff trainings were reviewed by LC for the 2023-24 QRIS year, the number of hours needed for educators and education director to be in compliance were not completed. — Visit: QRIS Monitoring visit — Action taken: Two of our team members have applied to earn specializations through CCR&R, which will provide many training hours. The Education Director and Program Director will review the center's upcoming 2025 training dates and schedule approved speakers for those dates. In addition to those actions, we will also seek to have our own trainings approved through Train TN so that teachers will get credit for attending them. We will help teachers choose appropriate self-paced courses through ProSolutions and SmartTeach/Teaching Strategies to complete on their own.

1240-04-01-.12 Health and Safety

During an announced QRIS visit while checking medications there was an Epi-Pen checked by that expired July 2024. — Visit: QRIS Monitoring visit — Action taken: The child has been on medical leave for several weeks and will return to our center the week of December 2, 2024. We will inquire about any medical updates and ask if it is still necessary for her to have an Epi-Pen available for use. If the Epi-Pen is still needed and is replaced, we will create a calendar reminder dated one month before the new Epi-Pen expires so that we can request a replacement in advance.
