The agency was not in compliance as there was not at least one staff member with current certification in pediatric First Aid and pediatric CPR on site at all times. — Visit: QRIS Monitoring visit — Action taken: The primary educator/owner plans to have all staff to receive infant/child CPR and first aid certification. Current staff are scheduled to receive CPR and first aid certification by CCR&R on February 21, 2026.

The agency was not in compliance a staff member did not have proof of required documentation. — Visit: QRIS Monitoring visit — Action taken: The primary educator/owner plans to use a checklist to ensure all required documentation for staff is on file prior to their start date.

1240-04-01-.11 Supervision

The agency was not in compliance when implementing the playground supervision plan. — Visit: QRIS Monitoring visit — Action taken: All staff have recieved coaching on name to face roll call at every entry point when transitioning to and from outside. This was reviewed with staff October 2025 and will also be reviewed by staff March 2026.

The agency was not in compliance as a staff member did not have documentation of a health exam within 10 calendar days of employment or work start date. — Visit: QRIS Monitoring visit — Action taken: Management required the employee to get a physical immediately. Management plans to use a checklist to ensure physicals are completed prior to employees starting.
