On Thursday, 05/14/26 a child in the Transition 1 room opened the door that is the direct exit outside and the educator heard the door opening and called out to the child who then ran around the front of the building. The agency failed to report a supervision lapse: On Wednesday, 05/13/26 a child in Transition 1 room climbed over shelving and exited the classroom and entered the staff breakroom, where the educator there called out to the classroom and let them know he was there, and returned him to the educator in charge of the classroom, that educator had been assisting with handwashing with her back to the classroom. — Visit: Complaint Investigation,Technical Assistance — Action taken: Educator was terminated for lack of supervision. The facility re-train educators on supervision and how to always maintain supervision during handwashing, diapering, etc.

In the 6 wk - 12 month old classroom the TA left the room to gather some items making the infant room out of ratio. The ratio is 1:4. When TA left the room the infant room was 1:5. This was verified by LC watching video. This agency self reported this incident. — Visit: Technical Assistance,Incident or Injury Investigation,Complaint Investigation — Action taken: The director reiterated ratios and reminded staff that walkie talkies are to be used to communicate needs that arise in the classroom to allow another staff member to help them get what they need. Ratios are already posted in rooms but the director will encourage teachers to show TAs where this information is located.

An educator fell asleep during naptime. There were 4 toddlers present and all were sleeping during this time. There was a 5 year old in the 2 1/2 - 3 year old room. — Visit: Complaint Investigation,Technical Assistance — Action taken: The director discussed the importance of following DHS rules and regulations with the staff members involved. The director reached out to the board members for guidance. It was decided to re-train all staff on DHS licensing rules/regulations, Board members will spend more time in the center and training the teachers. The staff member will be replaced the staff member as soon as possible. The employee will be written up and possibly replaced as soon as possible. Staff will be re-trained on DHS rules for combining and ratio.

All staff have been trained on the updates.

In response to a complaint video was observed. Upon observing video on 9-17-24 at 9:21 am, a child was left unattended on the changing table. — Visit: Technical Assistance,Complaint Investigation — Action taken: "You must remain at the changing station with child" has been added to the diaper changing table sign. This staff member was sent home on suspension pending the outcome of one on one training for safety and behavior management with CCR&R.
