1240-04-01-.12 Health and Safety

On 11/21/2026 while children were allowed to play in a hallway, a 20 month old and 21 month old went out of a door leading to the fenced in playground. The children were unsupervised on the playground for approximately 5 minutes. During a complaint investigation on 11/25/25, FS Pass and LC Spells reviewed video and educator statements of an incident from 11/21/25 where two 19 month old child toddlers were unsupervised. The children were playing in a hallway and entered a classroom with an open door. The educators did not notice that the toddlers had entered the classroom. They were in the classroom unsupervised for approximately 3 minutes. Because the hallway was not a safe place for the children to play and staff didn't follow safety protocols, 4 children were left unsupervised creating an unsafe environment. — Visit: Safety Plan Served,Follow-up visit — Action taken: There will be a review of active supervision policies. Effective immediately, hallways are not to be used as play spaces. Securing doors and access points-will ensure that classroom doors remain closed and latched when not actively used for supervised transitions. Update supervision policies to ban hallway play and require line of sight at all times. Enforce transition protocol. Monitoring will include daily supervisor walk throughs (am/pm) and daily video-spot checks.

Management didn't ensure the children were playing in a safe place and staff didn't follow safety protocols leading to 4 children being unsupervised. Management are responsible for insuring that staff are capable and trained and are responsible for an efficient operation of the childcare agency. — Visit: Safety Plan Served,Follow-up visit — Action taken: Management/supervisors will be re-trained in center policies for safety and supervision. They will in turn re-train and evaluate teachers for compliance. Investigate root causes and identify solutions such as environmental controls, supervision and positioning gaps, policy non-compliance, re-training of active supervision, restructure staffing and coverage so that all rooms will have identified openers and closers and ensure that the building is physically safe for all children. Daily monitoring will include physical walk throughs and video monitoring.

During a complaint investigation on 11/25/25 FS Pass and LC Spells reviewed video and educator statements of an incident that occurred on 11/21/25 where a 20 month old child and 21 month old child were unsupervised. The children were playing in a hallway, and went out of a door leading to the fenced in playground. The children were unsupervised on the playground for approximately 5 minutes. The classroom educator Ella Jones, combined her class with Charles Keith, Anita Morgan, and Alisha Morgan. Ella Jones did not provide the educators with a roster. When Ella Jones noticed two of her children were missing when she returned to take her children back to their classroom. Both children were found on the playground. — Visit: Complaint Investigation,Safety Plan Served — Action taken: Moving forward, children will play in designated licensed areas where they are seen and heard by educators present. All doors to classrooms will be closed. Teachers will have a roster of their classroom at all times and will perform name to face during all transitions. Schedules for outdoor and indoor gross motor play will be reevaluated as well as suggesting to use classroom by moving furniture out of the way.

1240-04-01-.15 Program Language and Literacy Development

On 11/25/2025, during an investigation of a supervision lapse, FS Pass and LC Spells reviewed video footage and documented incident descriptions from educators. While viewing video footage, we observed educator Ella Jones go to the playground to retrieve the children. She was walked quickly and took the children by the hand. The one and a half year old child fell when she grabbed his hand. She picked the child up off of the ground by his arm, lifting his feet off of the ground. — Visit: Complaint Investigation,Safety Plan Served — Action taken: Teacher(s) will be retrained on positive behavior management and safe physical interaction techniques. They will learn how to assist children in standing or moving safely without pulling or lifting by limbs. Will ask for training with CCR&R. Also, we will increase classroom observations for compliance.

On 8/26/2025, Licensing Consultant, Kathie Bennett, completed an Unannounced Visit to Chambliss Center for Children to serve a Safety Plan. The Safety Plan required verification that the buses contracted for transportation of school-age children had the required insurance coverage. The agency was unable to provide proof of insurance for the two buses contracted with Coretransit, LLC. On 8/22/2025, the owner of Coretransit, LLC, sent an email to DHS representatives admitting to transporting children without insurance on 8/6/2025 only. However, on 8/28/2025, Coretransit, LLC owner admitted to DHS representatives that the School-age children were transported to and from school from 8/6/2025 through 8/20/2025 without insurance coverage. The owner of Coretransit, LLC admitted that an insurance policy dated 8/7/2025 -8/7/2026 was given to Chambliss Center for Children and TNDHS Licensing despite the policy never being active. — Visit: Safety Plan Monitoring,Follow-up visit,Technical Assistance — Action taken: N/A

Kathie Bennett, Licensing Consultant, visited Chambliss Center for Children on 6/10/2025 for an Unannounced Complaint investigation. On 6/8/2025 at 5:15p.m., Linda Height, Educator, was providing supervision for two children (female 6years old and male 7 years old). Height left the children unsupervised in the classroom for 2 minutes, according to the video footage. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency has corrected this violation by installing monitoring equipment and retraining staff on supervision requirements.

On 10/9/2024, a 3-year-old child found a plastic sandwich bag on the playground with two over the counter pain reliever pills inside. The child put one of the pills in their mouth but spit it out onto the ground. Retraining of all staff regarding the importance of keeping all personal items out of reach of the children. 2.
