An educator served a child a food containing an ingredient in which the child is allergic. — Visit: Complaint Investigation — Action taken: The director met with all staff on 7/30/25 to discuss the two violations in detail. All staff were required to complete 2 trainings "Food Allergies: Recognizing Allergic Reactions and Meal Planning" and "Emergency Preparedness: Better Safe Than Sorry!" by Friday 8/1/25. This training have been completed. The staff member who served the food containing a food allergen to the child with the known allergy was terminated on the same day of the incident.

An incident occured and was not documented by Agency immediately containing required information and documentation was not provided to parent. — Visit: Complaint Investigation — Action taken: The director met with the educator that was present during the incident and discussed DHS licensing rules regarding reporting incident reporting and required documentation. The director also discussed licensing rules regarding providing parents with written documentation for incident reports. The director met with all staff on 7/30/25 to provide guidance and training on incident reporting and documentation. Training was given regarding providing parents with written documentation and filing the documentation in the children's files.

During an unannounced visit, Licensing Consultant (LC) Fox observed a small cloth in a crib with an 11-month-old sleeping infant. The cloth was immediately removed from the crib. The infant educators were unaware that the cloth was in the crib — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The director met with all infant classroom educators and reviewed the rules and regulations for safe sleep. The educators will ensure that nothing is in the crib before placing an infant in the crib for napping.

Ageney self-reported an incident of a child being left alone for approximately two minutes on the playground. The supervising educator failed to complete a name-to-face roll call before leaving the playground. — Visit: Technical Assistance,Complaint Investigation,Safety Plan Served — Action taken: The administrators will be reviewing the current safety policies and procedures and making changes as needed. Additionally, the agency will train all staff in supervising children and ensure that Name to Face Procedures are followed as required to ensure children are supervised at all times.
