During a safety plan monitoring visit on 5/15/2026, FS Pass observed the Lovely Lion, 3 year old class, transition to the bathroom. The educator did not do face to name roll call upon arriving at the bathroom. While FS Pass was in the hallway observing another class there was a 4 - 5 year old class left unattended for less than one minute. The educator left the class unattended to ask the office manager a question. The office was not in direct sight or sound of the children in the hallway. FS Pass observed the educator return and she took her class to the playground. On 5/15/26, FS Pass was conducting a Safety Plan Monitoring visit. FS Pass was observing the three year old classroom transition to the bathroom. Once at the bathroom 3 children sat in blue chairs and two children went into the bathroom. The educator was not in direct sight of the children in the bathroom for about a minute. During an investigation, LC Woods found that on 4/13/26, two educators and a floater left a 2 1/2 year old child unattended in the classroom bathroom area for approximately two minutes. The class transitioned to the playground while the child was in the bathroom. The Owner and Management have the ultimate responsibility for the safety and well-being of children in care by ensuring that qualified staff members are hired to work for the agency, and that after hiring all staff members perform in compliance with child care licensing requirements. — Visit: Complaint Investigation,Safety Plan Served — Action taken: N/A

During a complaint investigation on 11/25/25, LC Woods discovered that on 11/21/25, two educators left a 3-year-old child in the classroom unsupervised for approximately 3 minutes. The child was left behind when the class went to the playground. The educators did not notice that the child was left behind. 1240-04-01-.15 Program Language and Literacy Development

On 4/10/2025, A three-year-old female child came out of the bathroom and was asked to sit down on the wall. The educator grabbed the child aggressively and sat her down on the wall. Then, a three-year-old male came out of the bathroom and was asked to sit on the wall. The three-year-old male did not sit down and was getting up. The educator grabbed the three-year-old male by the arm forcefully and caused him to hit his head on the wall. He was checked out, and he did not have any injuries or bruises. He has not complained of it hurting. Juan was terminated from the job on 4/10/2025.
