    LC was investigating a self reported incident of a child being left in the classroom by alone during dismissal time; this was found to be a valid incident. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Director stated that she was going to put in to place an "end of the day" transition change that would hopefully make the afternoons easier to manager, and will help ensure children are no longer left unattended in their classroom. Director implemented this plan which includes designating specific educators to retrieve the bus rider children from their classrooms and take them to the bus. Other educators are advised to remain in their classroom with the children who are picked up by parents. Director and chosen educator or Assistant Director will go to each classroom and gather the children who stay for aftercare and take them to the aftercare classroom. Once the aftercare children are dropped off the Director goes back around to each classroom and uses the "SWEEP" method to ensure the classroom are empty before educators leave the classroom.

LC waited for the educators to recognize the child was in the restroom, but they did not LC had to alert the educators that the child was in the restroom with the kitchen play appliances. — Visit: QRIS Monitoring visit — Action taken: Agency will retrain educators in supervision. There will be modification made to the classroom to lower ration for safety purposes
