When children were transitioning off the transportation vehicle, the passenger log was not filled out. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Facility will provide each driver with blank bus logs-along with their assigned logs, so staff are able to write down which child enters the vehicle. Wesley House staff will also ensure before they depart from their route, that they have the correct bus log in their folder.

1240-04-01-.07 Criminal Background Check and State Registry/Records Review Procedures

A staff member was allowed to work on site prior to receiving background check clearance. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Staff has received conditional letter from DHS on staff. Currently waiting to receive full clearance. Wesley House will wait to receive clearance from DHS before staff begins working on site.

A child was found unsupervised in the hallway. The staff responsible were not aware the child had left the classroom. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The center has taken the following corrective actions to help avoid any further supervision lapses: The center has reviewed supervision policies with all staff. The center has implemented new rosters for special activity groups and reviewed the current roster use in normal classrooms. The center has rearranged schedules for small groups to be present in the hallways. The center will hire additional staff for supervision purposes.

A child was found alone and unsupervised in a hallway. Child care staff did not follow a clear procedure for tracking children. Management is responsible for ensuring that children are appropriately tracked and supervised. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The center has taken the following corrective actions to help avoid any further supervision lapses: The center has reviewed supervision policies with all staff. The center has implemented new rosters for special activity groups and reviewed the current roster use in normal classrooms. The center has rearranged schedules for small groups to be present in the hallways. The center will hire additional staff for supervision purposes. The center is working with CCR&R for supervision guidance and support.

1240-04-01-.07 Criminal Background Check and State Registry/Records Review Procedures

PS arrived on site for a scheduled QRIS visit. There was a staff member onsite that has not cleared his out-of-state background check and received state clearance to be on-site. This person was in a classroom with children and was sent home effective immediately and will return once the background check clears. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): The center immediately sent the employee home form the center and assisted with out-of-state background forms to complete the background check for the employee. No staff will be on-site without a form of background clearance letter.
