During an announced visit conducted on Tuesday, October 21, 2025, the Licensing Consultant (LC) reviewed staff training hours in TNPAL and observed that not all staff have completed the required annual training hours. LC spoke with the director and the director has ensured that the hours will be completed by 11/15/25. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The provider has ensured that a plan has been put in place to ensure that all trainings are completed timely.

1240-04-01-.08 Record Keeping
Licensing Consultant (LC) conducted an unannounced visit on September 30, 2025. During the visit, LC requested files of children that were a part of the program. The provider was unable to provide the requested files. 1240-04-01-.08 RECORD KEEPING. (1) General Record Requirements. (a) All records required by this chapter shall be maintained in an organized manner onsite, in a centralized location, or available electronically, and made readily available upon Department request. (2) General Children's Records. (a) The childcare agency shall maintain a file for each child in a central location within the childcare agency. (b) A child's records shall be kept for one (1) year following the child's leaving the childcare agency; however, the health record shall be returned to the parent/guardian upon request when the child leaves the childcare agency. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency has taken action to ensure that pre-k headstart files are never removed from the premises and are kept in accordance to the rules as all children files are. During an announced visit on 02/13/25, LC observed infants in the infant room watching Coco Melon's Wheels on the Bus program. For children less than two (2) years of age, use of electronic media and other electronic devices is prohibited. The infants in the classroom during the observation ranged from 4 months to 11 months. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The provider has agereed to additional training for staff to ensure they understand the media policy for children under 2 years old and are aware of additional resources for children under 2 years old.
