During the visit on March 23rd, 2026, Child Care Licensing Consultant Lexi Casteel was investigating an incident that was self-reported by the day care and reviewed video footage of a staff member yanking toys out of a child's hand and jerking him up while trying to get the child to clean up. The educator aggressively moved the child and used an aggressive tone. — Visit: Complaint Investigation,Safety Plan Served — Action taken: The provider terminated the educator immeditaely and will implement training on behavior managemenet and stress management.

1240-04-01-.09 Incident Reporting

During the visit on December 2nd, 2024, Child Care Licensing Consultant Lexi Casteel determined that the director and assistant director were aware of the staff member fall asleep and compromising supervision. The director and assistant director failed to report the incident to TDHS. The director of the agency is responsible for ensuring all incidents are reported to TDHS on the same day. — Visit: Complaint Investigation — Action taken: The director, assistant director, and staff will undergo incident/accident reporting training with CCR&R.

1240-04-01-.11 Supervision

During the visit on December 2nd, 2024, Child Care Licensing Consultant Lexi Casteel observed video footage while investigating a complaint of a staff member falling asleep while they should have been supervising children. Staff should always have direct sight of children and sound. — Visit: Complaint Investigation — Action taken: The provider, Little Learners At First Church, terminated the individual who was found to be sleeping while working with children. All staff will undergo a nap time supervision training.
