During a self reported incident investigation, it was determined that three children walked out of a play area, went up a stairwell and were without supervision for 2-3 minutes. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: All staff members at the agency will receive training in Supervision and administration will submit a supervision plan to DHS.

1240-04-01-.11 Supervision

During a complaint investigation, it was determined that a child was unsupervised for a small amount of time. — Visit: Complaint Investigation,Technical Assistance — Action taken: Staff will receive training in the areas of supervision and incident reporting.

1240-04-01-.15 Program Language and Literacy Development

While investigating a self reported serious incident it was determined that an educator used inappropriate discipline while handling a child. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: LC talked with Director about the facility's safe handling of children policy to which each employee has to sign off that they read and understand. LC told Director that she will be making TAM referrals to have CCR&R come out and give further guidance on proper behavior management techniques with young children. LC also discussed reviewing the Behavior Management and Guidance portion of Licensing Rules and Regulations with staff regularly.

Facility did not report a serious incident to a child's parents. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: LC discussed with Director that all incidents should be reported by the end of the same day with the child's parent(s) or guardian(s). There should be a written incident report and the incident should be verbally discussed with the family as well.

An incident ocurred a few months ago but was not reported to admin or the department until month's later. — Visit: QRIS Monitoring visit,Technical Assistance,Incident or Injury Investigation — Action taken: Center states they will conduct weekly check-ins with Director and educator.

1240-04-01-.06 Staff

New hire was left in a classroom unattended with a Toddler before their fingerprints were cleared. — Visit: Incident or Injury Investigation,Complaint Investigation — Action taken: Center stated, they have revised their onboarding process. In the future, we will ensure that all new hires have a criminal background clearance before they start. That means they will not shadow or be in the classroom at all unless we have their clearance.
