Educators failed to conduct roll call before leaving the grassy area and returning to the classroom. — Visit: Complaint Investigation,Technical Assistance — Action taken: Teacher's received technical assistance from LC, a message was sent informing teachers of the incident and corrective action plan, all teachers received additional training on conducting name to face and continued supervision of the classroom during transition times is being conducted by administration. The agency will continue to enforce the importance of name to face during transitions for children's safety, conduct training through CCRR for our orientations regarding name to face for all teachers, one on one training for the teachers who did not perform name to face to ensure they are properly doing roll call.

Agency was using an unlicensed outdoor play area that was not enclosed by a fence. — Visit: Complaint Investigation,Technical Assistance — Action taken: Only the playgrounds, which are fenced in and approved, will be used for outdoor gross motor time. Should approval be granted for other outdoor spaces, we will abide by the allowed areas to maintain the health, safety, and supervision of our students.

Two three (3) year olds were left unsupervised outside in a grassy area right beside the playground for approximately 2-3 minutes. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency has already begun additional training for all staff on the importance of supervision and plans to ask CCRR to conduct supervision training during orientation. The agency will continue to enforce supervision requirements during staff meetings and staff memos sent out weekly. New Hire orientation will continue to stress the importance of supervision and continue to require staff to sign a document stating they understand the importance of supervision after receiving training.
