    YMCA @ Granbery had three violations given, which caused three Corrective Action Plans to be generated. After numerous reminders the Corrective Action Plans were not completed by the agency. — Visit: Follow-up visit,Safety Plan Monitoring,Technical Assistance — Action taken: LC has sent several emails with CAP numbers explaining they need to be filled out and also that once CAP was filled out with the wrong information and needed to be corrected and this was not completed after numerous reminders. LC also put in a TAM referral on 3/20/2026 to have CCR&R help them get these CAPs filled out.
