   During an unannounced visit to investigate an incident after speaking to educator involved it was determined that a Name to face roll call was not done before educator left the classroom to return to the playground which resulted in a 4 year old child being left in a classroom unattended for approximately 2 minutes. — Visit: Incident or Injury Investigation — Action taken: Following the incident in which a child was left unattended, the staff member responsible was immediately placed on administrative leave pending investigation. Parents were notified the same day, and a report was submitted to State Licensing as required. An internal investigation was completed in coordination with the State Licensing investigation. Appropriate personnel actions were taken based on the findings. Name‑to‑Face Training — All teaching staff will complete a full refresher training on the Name‑to‑Face active supervision procedure through CCR&R. Training will be documented and added to each staff member's personnel file. Supervision Protocol Review — All staff will review the center's Supervision Policy, including sight‑and‑sound supervision, zone coverage, and transition procedures. Staff will sign an acknowledgment form confirming understanding. Administrative Monitoring — Administration will conduct random supervision audits for the next 30 days to ensure compliance with Name‑to‑Face and transition procedures. Findings will be documented and reviewed with staff.
