1240-04-01-.11 Supervision

During an incident investigation it was confirmed that a child was left unsupervised in a classroom. — Visit: Safety Plan Monitoring,Incident or Injury Investigation,Technical Assistance — Action taken: Director told LC that when educators line children up to leave the classroom one educator is supposed to be at the front of the line and one at the end of the line. The educator who is at the end of the time is supposed to do a "sweep" of the room, checking all centers and bathrooms (if bathrooms are located in the classroom) to make sure all children have made it to the line. The "sweep" procedure combined with "Name to Face" roll call in intended to prevent children from being left unsupervised. Director will be reviewing procedures for transitioning with staff, and CCR&R referral will be made.

1240-04-01-.11 Supervision

During an incident investigation at the facility it was discovered that educators did not perform "Name To Face" roll calls while transitioning from the classroom to the playground. — Visit: Safety Plan Monitoring,Incident or Injury Investigation,Technical Assistance — Action taken: LC discussed with Director the importance of educators conducting "Name to Face" roll call at every transition. Director is going to talk with her staff about ensuring "Name to Face" is conducted any time the class transitions from one space to another. CCR&R referral will be made to include training on handling transitional periods.

During a complaint investigation, LC was made aware that there were incidents that were not reported to DHS/DCS. — Visit: Complaint Investigation — Action taken: Make copies of the DHS rule on being a mandated reporter and have staff sign off on it All staff attend a CCRR training on being a mandated reporter

During an unannounced visit to investigate a self reported incident, after interviewing the center manager and educator who witnessed incident it was determined that inappropriate discipline was used on a 2.5 year old child. — Visit: Incident or Injury Investigation — Action taken: The remaining staff will attend a supervision training conducted by CCRR. The staff member was terminated

1240-04-01-.12 Health and Safety

During an announced QRIS visit, when LC was checking medications the LC discovered there was an Epi- Pen in the classroom emergency bag that expired November 2024. — Visit: QRIS Monitoring visit — Action taken: The current epi-pen was moved to the emergency bag. The expired epi-pen was returned to the parent at pick up on 03/04/25. The agency is adding a checklist for checking the medications monthly to make sure they are in the right place and not expired. The agency is also adding medications to our allergy list with the expiration date added too.

During an unannounced visit to investigate a complaint after interviews with educators, it was determined that an educator was using a loud tone of voice when directing the children to other activities throughout the day. — Visit: Complaint Investigation — Action taken: Center Director met with educator to discuss her loud tone of voice and the proper level and tone to use while in the classroom and/or in the presence of children. Center Director utilized the company's documentation processes and progressive discipline plan. Center management will do frequent observations (announced and unannounced) of this educator. Management will also look into a training for all staff to participate in that discusses proper classroom communication in ECE.

1240-04-01-.13 Food and Food Service

During an unannounced visit to investigate a complaint,after interviews with educators it was determined that children in a classroom were not being given enough time to eat lunch or snack. The children also were not allowed to get seconds if they have not tried all the fruit, meat or vegetables on their plate. — Visit: Complaint Investigation — Action taken: Center Director met with educator to discuss the concern about not allowing children seconds and not giving them enough time to eat. Licensing rule 1240-04-01-.15-5C (pg 59) was shared with her. Center Director utilized the company's documentation processes and progressive discipline plan. Center management will do frequent observations (announced and unannounced) of this educator, specifically during mealtimes. Management will also look into a training for all staff to participate in that covers proper mealtime processes.

During an unanounced visit to investigate a complaint, after interviewing educators it was determined that on more that one occasion children were on the toilet up to or longer than 30 minutes which is a violation of licensure rules. Children were also not alllowed to get up from toliet when they asked to get up. Children can't sit on the potty longer than 5 minutes at a time. — Visit: Complaint Investigation — Action taken: Center management will do frequent observations (announced and unannounced) of this educator. Management will also look into a training for all staff to participate in that discusses proper classroom management in ECE. Center Director met with educator to discuss this occurrence. She explained that it was at a parent's request for potty training, she did it one day and felt horrible about it, and told the parents we couldn't use this technique anymore. CD reinforced that the max time is 5 min per licensing. Center Director utilized the company's documentation processes and progressive discipline plan.
