An incident/injury form was not completed and placed in the child's file as required. — Visit: Complaint Investigation,Technical Assistance — Action taken: Staff will utilize CCRR training in the areas of incident reporting and documentation, and follow licensing rules on reporting and documentation. The agency staff will report incidents requiring medical attention to DHS.

During an incident/injury investigation on 1/07/2025, License Consultant (LC) Tatoyra Grady discovered the child that was involved in the incident was not signed in and signed out on the date the incident took place which was on 01/06/2025. — Visit: Technical Assistance,Complaint Investigation — Action taken: The agency corrected the violation with the following action(s): A sign is posted reminding parents/guardians to signed their child(ren) in/out. There will be some form of monthly communication to remind parents /guardians to sign their child(ren) in/out. If parents/guardians fail to sign their child(ren) in/out, they will be called to return on the first offense. On the second, child will be withdrawn from enrollment. The agency will seek futher guaidance from CCR&R on the issue.
