During an unannounced visit on July 1, 2026, the agency was cited a violation for Incident Reporting. An incident occured involving a three-year old child on July 30, 2026 in the Pre-k 3 classroom. The incident was not reported to the parent by the end of the day. In addition, the agency failed to document the incident and provide the parent with a report. — Visit: Complaint Investigation — Action taken: The director reviewed rules, regulations, and policies with staff again. Let employees know just how important it is to document everything and how important communication is with the parents.

There was a child who was under the age of 6 weeks of age enrolled and present in the infant classroom. Children under 6 weeks of age are not permitted to be enrolled in licensed child care. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The director will be responsible for enrolling children and dates of birth will be checked prior to enrollment. The was an infant who was not 6 weeks old was present in the infant classroom. The agency director, Elizabeth Smith self-reported a 4 year old child being in the bathroom across the hallway from her classroom without supervision. — Visit: Complaint Investigation,Incident or Injury Investigation,Technical Assistance,Safety Plan Served — Action taken: The agency received on-site support and training by Child Care Resource and Referral for Active Supervision of Children during transitions. Agency will require a floater to assist with transitions to the restroom.

The annual report was not submitted until after the license expired. The agency reassigned responsibility for completing the report.

A second eductor will be required for all transitions, and an immediate sweep of the playground will be conducted.

The agency director will review policies for Reporting Child Abuse and Neglect and will work with partnering agencies to schedule an in-service training for all educators.

During an unannounced visit conducted on March 3, 2025, to investigate a complaint the agency was cited for forcing a child to stay on their mat/cot and not allowing the child to have a quiet activity or book. — Visit: Technical Assistance,Complaint Investigation,Safety Plan Served — Action taken: The agency will be reviewing all rules related to napping and sleep practices. Children will be allowed to have a quiet activity if they are not napping or wake up early.

During an unannounced visit on September 13, 2024, Licensing Consultants (LC) Fox and Taylor observed the Tiny Tots/Next Steps classroom entering the toddler playground at approximately 9:30 a.m. Ten children and two educators entered the playground. One educator counted the children as they entered the playground; however, name to-face was not done. Educators state they usually do name to face, but the lead teacher was still in the classroom and had the tablet. — Visit: Order of Civil Penalty Served,Other — Action taken: The educatorS received one on one training on Name to Face procedures with Child Care Resource and Referral Health and Safety Coach. In addition the director has scheduled trraining for all educators.
