Child was left unattended in a classroom for a 1 minute 30 seconds. Educator counted the children and counted another child not in her class, causing a miscount. Educator did name to face at final location realizing they only had 6 children. Educator returned to the classroom to get the child that was left behind. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: DHS gave two violations to othe center. DHS LC disucssed the lack of supervision and name to face policy. A safety plan will be done to ensure staff has name to face training and will follow-up with visits to ensure that the new transition plan in the afternoon is working.

LC Ratliff interviewed educator and saw video. LC gave the center two violations and put a safety plan in place for educator and staff to receive additional training on supervision and name to face.
