During an unannouced self reported incident investigation it was discovered that a employee left for the day and di not communicate this with other educators resulting in a child being left unsupervised. It was discovered during investigation that a child was left unattended in the commons area while two other educator took the infant/toodler on a stroll around the commons area on the bottom level. — Visit: Incident or Injury Investigation — Action taken: N/A

It was discovered during the investigation that although the parent was informed there was not a written incident report filled out for the parent to sign. — Visit: Incident or Injury Investigation — Action taken: CCR&R will provide ongoing observation for classroom. Internal policy and procedures will be reviewed. There will no longer be combining in the commons area, combining at the 1st hr of day and last hr of the day will take place in classrooms
