During an unannounced complaint investigation visit on September 22, 2025, Field Supervisor Tiffany Felton observed in the infant classroom an infant asleep in the crib with a bib around her neck. According to the child's daily activity chart, she had been asleep for 5 minutes when FS Felton walked in and saw the bib and asked the educators to remove it immediately. The two educators were sitting on the floor with the other 5 children. — Visit: Complaint Investigation,Technical Assistance — Action taken: A TAM referral has been completed and the center has started their process to correct the issue.

The infant classroom had an infant asleep and a (SIDS) Sudden Infant Death Syndrome sleep check was not completed every 15 minutes as required. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: LC has completed a TAM referral for training for infant/toddler educators and supervising staff on SIDS checks. A follow up viist will also be done. The center has had meetings with staff and they are following up on their plan with staff.
