During an unannounced visit to investigate a complaint, after watching video of the incident and reviewing the name to face log from the classroom, it was determined that educators did not conduct a name to face roll call when entering the classroom. The class was coming from the playground. — Visit: Complaint Investigation,Safety Plan Served — Action taken: The agency will have a training scheduled by CCR&R to provide to all staff and thoroughly review the name to face sheets. Reminders about name to face roll call sheets will be created much larger to replace the ones currently on all doors that the children enter/exit. The program director will make several random checks throughout the day to ensure that all name to face sheets are getting completed correctly and accurately. Both educators were immediately terminated due to the failure to complete the name to face roll sheet despite the numerous trainings that have been provided by different resources.

During an unannounced visit to investigate a complaint, after LC watched video and spoke to director it was determined that a child was left unattended outside on the walkway that leads to the classroom for three and a half minutes until the educator opened the door and brought him inside the class. The class was coming in from the playground. — Visit: Complaint Investigation,Safety Plan Served — Action taken: Both educators were immediately terminated due to the failure to complete the name to face roll sheet despite the numerous trainings and conversations that have been held. The agency has requested the licensing consultant to do a referral to CCR&R for training on supervision and completing name to face sheets. Reminders about completing the name to face sheets, that have been placed on all doors that children enter or exit, will be made much larger. The program director will complete random checks through the day, as a constant reminder to staff, to view each classrooms name to face sheets. The parent of the child who was left outside, has volunteered to come speak with the staff to share the concern and what this does to a parent who is affected in this type of situation.
