1240-04-01-.09 Incident Reporting

During a complaint investigation it was revealed that law enforcement was contacted by the agency and was not followed up with an incident report written by the agency. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency took training on policy and DHS Rule in category Incident Reporting. The agency plans to continue ongoing monitoring to ensure they are reporting all required incidents and contacts. In addition, CCR&R will give Technical Assistance Training and the agency will receive monthly reminders about incident reporting rules from compliance connection.

1240-04-01-.11 Supervision

During a transportation check children were allowed to enter the bus with out an adult on the bus. The bus driver corrected the educator stating "you can not let them on the bus until I am on there". There were three children on the bus while educator and bus driver were outside the bus with out proper supervsion — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Metropolitan Action Commission Center Managers, Early Education Manager, Compliance Coordinator, and Administrative Personnel will continue ongoing monitoring. CCR&R will be conducting training in the area of supervision and transportation.

During an announced QRIS LC was able to see a loading procedure. Children were allowed to enter the bus without an adult on the bus. During investigation it was determined that an educator made supporting staff and admin aware of an incident that involved improper discipline of a child, parent or guardian was not made aware of this incident no later than the child's release to the parent/guardian or authorized representative on the date of occurrence. — Visit: Incident or Injury Investigation — Action taken: N/A

During an self reported investigation it was discovered that an educator used improper discipline techniques in order to gain control of the child. — Visit: Incident or Injury Investigation — Action taken: The entire staff will be retrained on the DHS Rules and Regulations 1240-04-01-.15 and Metropolitan Action Commission Center Managers, Early Education Manager, Compliance Coordinator, and Administrative Personnel will continue ongoing monitoring to ensure compliance.
